Richland County restaurant inspections: Feb. 7 critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health on Feb. 7:

Hong Kong Buffet, 331 N. Lexington Springmill Road, Mansfield, Feb. 7. Person in charge (PIC) did not ensure employees are properly maintaining temperatures of TCS foods during cold storage (critical repeat, corrected). Observed multiple areas where refrigerated TCS foods were not properly cold held at 41 degrees or less. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Multiple critical violations observed. Correct by Feb. 13. Written procedures and plans are not maintained and implemented (critical, corrected). Facility not following time in lieu policy as no time stamps observed.

Observed washed, cut broccoli stored in box used to previously hold raw chicken and leading to potential cross-contamination. PIC discarded box and placed broccoli in prep sink are to be rewashed. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed multiple pans of sauces, washed raw peppers and onions, mushrooms, and broccoli along make line in back kitchen area not covered/protected from possible contamination.

Observed multiple foods in walk-in cooler (sprouts, cooked chicken, raw chicken) uncovered/unprotected from possible contamination.

Chlorine sanitizing solution at incorrect temperature and/or concentration (critical repeat, corrected). Sanitizer at mechanical warewashing machine observed at 0 ppm. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected). Observed vegetable peelers in clean storage in kitchen with visible residue. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed knife sharpener, vegetable peeler and scissors used to open food products stored on shelf in hibachi area to contain visible dried food residues. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed visible residue on raw chicken mixer. Improper cooling of TCS food (critical repeat, corrected).

Observed cooked ground pork stored at room temperature to have internal temperature of 60 degrees. PIC discarded approximately 10 pounds of pork. Observed multiple pans of sushi with internal temperatures between 47-48 degrees and crab meat salad with internal temperature of 48 degrees. PIC placed in walk-in cooler to bring down to 41 degrees or less. TCS foods not being cold held at the proper temperature (critical, corrected). Observed multiple bottles of dressing (Italian, French & Thousand Island) and individual packets of whipped butter all indicating need for refrigeration after opening stored at room temperature in buffet area. PIC voluntarily discarded listed products.

Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed eggs made the day prior not date marked. Improper use of time as a public health control - 4 hours (critical, corrected). Observed sushi on buffet without time stamp. Working containers of poisonous or toxic materials not properly labeled (critical, corrected).

Observed spray bottles of bleach water in hibachi area without label. Food contact surfaces not easily cleanable (critical). Observed coating coming off on inside of rice bowl and no longer easily cleanable. Resurface or replace by Feb. 13. Equipment and/or utensils improper construction (critical, corrected). Observed cardboard box used to store cut, washed broccoli. PIC removed product and discarded box. Equipment and/or utensils improper construction (critical, corrected). Observed non food grade container used to store food in reach-in cooler. No air gap or approved backflow prevention device on the plumbing system (critical). Install backflow device or remove continuous pressure hose by Feb. 13. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed no air gap from drain hose of bulk ice machine at floor drain. Correct by Feb. 13.

Donatos Pizza, 855 Lexington Ave., Mansfield, Feb. 7. Insufficient air gap between the flood rim and the water supply inlet (critical repeat). Observed no air gap in the storage room behind the pop machine. Correct by Feb. 13.
